When it comes to selecting a top-notch surgeon or specialist you have to make a sound decision that is based on finding the absolute best doctor for you and it should not be limited by anything, least of all, your physical location.   I am fortunate to have patients come to see me from across the country and  other parts of the world.  After doing research they found me to be a good match for them based on my education, specialties, and fully accredited surgical facility.  What I admire about these patients is that they made a decision that was best for them but also one that was not based on their location.  They were not at all put off my the logistics of getting here and getting the best possible care for themselves.  As a matter of fact, some of my patients in and out-of-town treat their surgery and recovery as a personal vacation; an investment in themselves as a whole.  They take a few days and relax and recover at The Ritz Carlton in Buckhead; come in for their post operative skin care treatments at Oculus Skin Care Centre, order room service, and take a load off for a few days.  They make the most of their time and give their all to getting back to themselves quickly but not without a well deserved dose of R&R.We are lucky to be living in an age where technology is always one step ahead of us and communication is a very large part of how we live our day-to-day lives.  The days of waiting until we got home to call someone back on the home phone are gone.  We can simply plug a question into our computer or smart phone and have the answer in less than a second.  Some find this flux in technology impersonal and fear that something has been lost but I beg to differ.  In a field such as plastic surgery where a personal connection is key, clear communication is vital and distance can be a major barrier when it comes to selecting a surgeon and consulting about surgery.

Typically a patient will come to meet with me for a consultation and we will get to discuss their concerns in person.  For some of our out-of-town patients this is not a reasonable option.  In an effort to further extend the reach of my care from my office in Atlanta, I offer Skype Consultations for our cosmetic patients that are not able to travel to Atlanta to meet with me in person.  There is a small fee for this service.  This consultation will be considered a first step in the consultation process; followed by an in person consultation where a patient may decide to move forward with surgery.  It is a great forum for me to meet with a patient and discuss their concerns and goals and also provides a opportunity for my patients to ask questions specific to them and get a feel for my personality as well.

Contact my Patient Care Coordinator, Jennifer, at 404-256-1500 Extension 20 or jenniferm@oculusplasticsurgery.com to schedule a convenient time  for you to meet with me on Skype.  She will get some basic information from you and collect your $75 fee to reserve your appointment time.  Please allow at least 30 minutes for the consultation to take place.  There needs to be enough time to establish a computer connection and for Dr. Cole to meet with you and discuss your concerns in detail.

Prepare for your consultation

Sign up for an account and download the newest version of Skype onto your computer.  The service is free and very easy to use.  Add the contact name drchipcole to your list of contacts.  You may even want to do a test run with a friend to make sure that everything is set up and working before the day of our consultation. 

Download new patient forms from my website and fax (404.256.2006) or email (jenniferm@oculusplasticsurgery.com) the completed forms back to my office, prior to our consultation, so that I can thoroughly review your medical history.

Take a few high-resolution pictures of yourself, better yet, enlist someone to help you.  You want to take pictures of your specific areas of concern,  and some of the entire face from the front and side.  Try to fill the entire frame of the picture with your face.  Skype is a great communication tool but the image clarity is not always up to par so the pictures help me to see the fine details of your facial structure for reference during your consultation.  Email (jenniferm@oculusplasticsurgery.com) your pictures to my office prior to our consultation so that I can review them as well.

The Day of Your Skype Consultation

Jennifer, my Patient Care Coordinator, will establish the connection for the call and chat with you for a few minute to make sure that everything is working correctly.  This is a great time to make sure that you are in pleasant lighting and that your camera is steady and that you are well placed so that I  can see you clearly as we chat.  She may ask you to to adjust the way you are sitting in order to see you best during our consultation.  It’s best to be  facing a window or lamp.  You do not want to have a light source behind you as it will be difficult to see you well.  I will then come in, meet you and start the consultation.  It’s a really good idea to have some questions written down prior to the consultation that you may want to ask or you can use them to guide you through the consultation.  Sometimes, in the moment, patients forget what we would like to ask and then think of all the really great questions that we had 30 minutes too late.

Following the Consultation

Following the consultation you will be emailed a quote for the procedures that were discussed during our Skype consultation.  Please note that the procedures and quotes may change once I evaluate you in person.  Even with the best pictures and a Skype consultation taken into consideration nothing is better than seeing a patient with my own eyes and being able to evaluate the skin and facial structure in person.  Jennifer will touch base with you several times as you make your decision about surgery.  Please feel free to discuss any questions or concerns that you may have with her as you come to a decision.  She keeps me up to date on your case as you communicate with her. 

At this point you may decide that you would like to make a trip to Atlanta to meet with me in person or feel like you are ready to move forward with surgery.  We will do everything possible to make your time efficiently spent in Atlanta and while in our office.  All appointments will be made according to your schedule and within your travel time in Atlanta to the absolute best of our ability.  Your post operative care will be tailored to your location, schedule,  as well as your needs.  If coming back to Atlanta is going to be a difficult after surgery, I will do everything possible to come up with a post operative care program that works  for you.  This may mean an additional meetings on Skype or having you periodically send in high-resolution pictures.  Be advised that in some cases a trip to my office in Atlanta may be the best option for your care.

Surgery is a huge decision and should be one that you spend a good amount of time researching and thinking about.  What I don’t want is for your decision to be skewed, in any way, by where you are located and the choices of surgeons that local to you.  A Skype consultation is a great way to have an Oculofacial Plastic Surgeon visit you in your home. Who says doctors don’t do house calls anymore?  I look forward to Skyping with you soon!

Botox® is a wonderful injectable product that is designed to relax targeted muscles in the face that cause expression.  As we get older, our ability to bounce back is slowed and that is evident in every part of our body and the skin is no exception.  Years of making the same expressions begin to take shape as deep lines on the forehead and a furrow between the eyebrows.  Botox®  relaxes expression lines and smoothes out targeted areas in a brief office procedure with results lasting for several months.

Glabella Treatment, Actual Before and After Results

Patients begin to notice the effects of aging as early as their twenties.  They begin to notice small fine lines around the eyes in the form of crow’s-feet and a brow furrow.  In some cases great skin care and some advanced non-surgical treatments at Oculus Skin Care Centre is all a patient needs to get their skin back up to par.  When a patient has tried these techniques with no avail or prefers a more direct approach I may suggest Botox® Cosmetic to address these expression related concerns.  Botox® is a safe option for patients as young as 18 years of age or older.  There are two basic types of wrinkles: dynamic (muscle) and static (skin at rest).  Botox® is a prefered choice  for treatment when a patient’s primary concern is expression lines; not to be confused with lines that remain on the face when the muscle is at rest.  Although Botox® can help relax these standing lines on the face, a more effective choice to treat deeper lines and wrinkles at rest is an injectable filler product that increases volume in that targeted area, smoothing out the wrinkle more fully.  Areas that are frequently treated are the forehead, the glabella ( the area between the eyebrows just above the nose,  known as ‘the 11s”), and crow’s-feet.  There is also a technique I call “The Botox® Browlift” where a small amout of Botox® is injected into the muscles, just under the brow, that pull the brow down while keeping the muscles that pull the brow up completely mobile providing the patient with a beautifully arched brow and a subtle lift to the eyelid.

Treatment with Botox® is very quick and virtually painless.  During your consultation we will discuss your concerns and I will examine the muscles in the targeted areas that you wish to have treated.  I may ask you to make an expression such as narrowing your eyes, frowning or rasing your eye brows to get an idea of muscular strength in those areas and the severity of the expression lines being treated.   Taking all of this into consideration, together we will decide an appropriate treatment plan.  The product is prepared and I will have you relax as I inject the targeted areas with a very small needle.  Patients describe the procedure as a few small pinches.  After a few minutes your treatment will be complete.  You may have some immediate swelling which typically subsides in about 20-30 minutes.

It may take a few days for the effects of the treatment to take effect and this varies by patient.    For the first 24 hours you should avoid vigorous physical activity and take care not to press on or massage the treated areas.  As with any injection there are risks to consider such as tenderness, swelling and bruising at or around the injection site. If any bruising does occur it typically resolves itself within a few days. Some patients put ice on the area immediately after the procedure to calm any post-procedure swelling or discomfort.  With that said there are some things that you can do to help you achieve optimal results even before you have your procedure.  I caution my patients that using products that thin the blood which includes but is not limited to aspirin, products that contain aspirin and some dietary supplements can increase the likelihood of bruising and suggest discontinuing use* of these products for two weeks prior to your procedure. Taking a combination of supplements called Arnica Montana and Bromelain for a few days before and after your procedure can help speed healing and help the body to resolve any bruising faster while providing an anti-inflammatory effect.  These products are offered in a kit and can be purchased at Oculus Skin Care Centre to be used in conjunction with any surgical procedures to help in this way.

Whether your surgery is medically necessary or cosmetic in nature, next only to selecting a qualified surgeon with whom you are comfortable the next most important thing that you can do to get the best result from any type of surgery is to stop smoking: immediately. It has been researched for years and proven that smoking is not a healthy choice for your body.  There are various reasons to decide to stop smoking and we are only going to discuss one: to heal better and get a better result from surgery.

Cigaretttes contain a variety of ingredients including Nicotine, Carbon Monoxide, Hydrogen Cyanide and Ammonia to name a few.  A complete list of components of cigarettes and chemical released in cigarette smoke can be found hereNicotine is a vasoconstrictor and increases platelet adhesiveness both of which can reduce the amount of oxygenated nutrient rich blood that reaches the delicate post surgical areas that are healing.  Nicotine hinders the  production of red blood cells which deliver oxygen, fibroblasts which aid in wound healing and the formation of healthy scar tissue, and microphages which are a white blood cell that helps to combat infection.  The effect it has on the fibrobalsts has additionally been linked to excessive scarring or the over formation of scar tissue in some patients. 

If you are reading this blog, and are a smoker who is considering plastic surgery it would be wise to stop smoking.  Most doctors, including myself, recommend the cessation of smoking for a minimum of two weeks prior to surgery and for several weeks after surgery if not entirely.  I have noticed that incisions in smokers never heal as beautifully as they do in non-smokers.  Aside from the effects smoking has on your ability to  heal it also increases your chances of having surgical complications including infection and can reduce the longevity of your post surgical results by half. 

A healthy body heals better.  With elective surgery you have the luxury of choosing the date of your surgery so you have the time to make a difference.  Please keep in mind that if you suffer a trauma, require non-elective surgery, or sustain an injury to your body cigarette smoking will affect how you heal.   If you wish to continue smoking please remain in honest communication with your surgeon.  There are a few ways to increase the success of surgical procedures in smokers including specialty treatments with hyperbaric oxygen and vasodiolating creams.  For your safety your surgeon may decide that surgery is not a viable option for you at this time if you continue  to smoke.  Make a decision to become more healthy and contact your primary doctor to discuss a plan to stop smoking today and make you focus a beautiful new you!

darkcirclesPatients come in to see me all the time with complaints of darkness and deep ruts under the eyes making them look and sometimes feel older than they are. This is something that can be treated non-surgically during a quick office procedure and have you looking and feeling better about your appearance in as little as a lunch hour.

Dermal Fillers are an injectable product that can be used on the face, neck and even the hands to improve the appearance of wrinkles and restore areas where there is a volume loss. Sun protection goes far beyond simply applying a protective lotion before sun exposure. With environmental factors changing the way the sun affects us there are a variety of products on the market to help you protect your skin more thoroughly. Use a few of the following products on your outings and  you will keep your skin looking even better than using a sunblock alone.

Glamorous Sun Hats are a wonderful way to dress up an outfit or bathing suit and a great way to protect the precious skin on your face and neck while soaking up the rays. They are also a great addition for gardeners who get a ton of sun while digging in the dirt. Sunglasses are stylish and serve a very important role in sun protection. First, they protect the eyes from the devastating effects of the sun. Sun damage is a real concern when it comes to the precious tissues that make up and surround the eyes. Secondly, wearing a great pair of sunglasses prevents squinting which can form fine lines around the eyes. The eye area is one of the first places on the face where we notice the signs of aging showing as early as your 20′s.

Sun protection garments  are a fairly new market for the general consumer but have been used in water sports for many years. Most of these garments, which are available in just about every style that you can imagine, provide an impressive SPF 50 or greater. They are light weight and can be worn in the water or while enjoying outdoor sports and other activities. The benefit being that there is no need to reapply so it is a great option for extended periods in the sun and especially for children. If you are a do-it-yourself-er you can use a wash-in product  to add an SPF 30 to any clothing that you wear outdoors. All clothing provides a certain amount of sun protection but the sun can penetrate regular garments leaving you open to sun damage.

So, take the extra step and protect all of your skin with fashionable items that provide the function of great sun protection, because taking care of yours skin is always in style.

An "alternative" to addressing aging concerns around the eyes

When I sit down with a patient and listen to their concerns about their eyes and what’s going on as they get older, I realize what amazing things we can do today to actually reverse aging and take years off the face. There’s no debating the topic, surgeons agree that the earliest signs of facial aging show up in the eye area and those unwelcome changes are usually very dramatic! Eyelid surgery is among the “top 5” procedures in demand today.

My patients express concern over not just how they think they look but on how others say they look as well. Comments such as “you look so tired…are you getting enough rest? Or “you look sad all the time…are you o.k?” Or “why are you so angry all the time?” These kinds of comments drive both male and female clients into my office asking what can be done about the bags under their lower eyes, the hooded upper eyelids that are hindering proper vision and excess fat that has accumulated in those puffy under-eye bags.

Over the past decade I have seen many patients that have had “eyelid tucks” and have not seen the improvement they expected. Sadly, these patients, as do many that I see, require a multi-layered rejuvenation approach that takes into consideration not just the upper eyelid skin, but the area around the eyes. The crow’s feet, under eyelid bags or hollows, a harsh brow, or hanging skin are all areas to be considered in addition to the excess upper eyelid skin. For this reason I pioneered the Eyelight™ Blepharoplasty procedure which employs a laser and endoscope to rejuvenate both the upper and lower eyelids, brow and cheek area. The laser repairs and restores the facial tissue to its original position and texture  while restoring proper distance between the brow and lashes and a more youthful appearance. There are no visible incisions and no stitches with this procedure! This is an ultra precise procedure that achieves  natural looking results and puts the twinkle back in your eyes!

I have nearly as many men coming in these days as women because men today are more health conscientious and understand that appearance counts when it comes to workplace politics. Looking good can make or break a career in many cases and looking and feeling great boosts your self-confidence.

I don’t recommend surgery unless it’s necessary because there are some alternatives, but the age and condition of each patient dictates what will and will not get the desired results.
